BPFS · Back Pain Functional Scale

12 items · 4 min · 0–60 · higher is better

1 Before you use it

What it is

Twelve functional activities rated for difficulty — from the same group that produced the PSFS and the LEFS, and it shares their design logic.

Higher is better here, unlike most back measures.

When to reach for it

When you want a function-positive measure rather than a disability-negative one. Some patients engage far better with a score that goes up as they improve.

2 Giving it

Scoring

Each item 0–5, summed to 0–60. Higher means better function — the opposite direction to the RMDQ and Oswestry, and an easy one to record backwards.

What it doesn’t measure

Pain, fear, or work risk.

3 Reading the score

Interpreting it

Interpret against baseline.

Is the change real?

No verified MCID or MDC is carried in our library for this measure, so the number on its own cannot tell you a change is real. Compare it to this patient's own baseline and say what changed functionally.

Student trap. Recording it in the same direction as the RMDQ. This one goes up when they get better.

Official source: null. We don't host the form.

This is a learning tool. It does not diagnose patients and does not replace your clinical educator.
