What it is
A five-item cluster for identifying hip osteoarthritis in patients presenting with hip pain: squatting as an aggravating factor · lateral hip pain on active hip flexion · scour test with adduction producing lateral hip or groin pain · pain on active hip extension · passive internal rotation of 25° or less.
When to reach for it
Any hip or groin presentation in an older adult, and any time you are trying to separate hip OA from a lumbar or gluteal source.

Scoring
Count the positives. The value rises with the number of criteria met.
What it doesn’t measure
It does not exclude hip OA, and it does not replace imaging where imaging is indicated. It also says nothing about the other things that produce lateral hip pain.
3 Reading the score
Interpreting it
No verified likelihood ratios are carried in our library and none are printed here — read Sutlive et al. 2008 for the figures.
The item worth internalising regardless of the cluster: limited passive internal rotation. It is the single most consistent early finding in hip OA and it is the one students forget to measure.
Is the change real?
Student trap. Skipping passive internal rotation because the hip is sore. It's the highest-yield single measurement in the cluster.
This is a learning tool. It does not diagnose patients and does not replace your clinical educator.
