1 Before you use it
What it is
Metadata only. Twelve items specific to shoulder instability rather than pain — how often the shoulder gives way, how much it limits activity, how much it is on the patient's mind.
When to reach for it
When it appears in surgical correspondence. For your own instability baseline use WOSI, which is more granular and has an emotions domain.
2 Giving it
Scoring
Twelve items, 0–4 each, 0–48. Higher is better in the current scoring; an older reversed convention exists.
What it doesn’t measure
Apprehension on examination, or generalised hypermobility. Score Beighton separately.
3 Reading the score
Interpreting it
Is the change real?
Student trap. Assuming it's the shoulder score with instability items added. Different instrument, different items, not interchangeable.
Licensed through innovation.ox.ac.uk. We don't host the form.
Use instead: WOSI — same construct, no licence.
This is a learning tool. It does not diagnose patients and does not replace your clinical educator.
