ASDAS · Ankylosing Spondylitis Disease Activity Score

5 items · 3 min · Continuous, approx 0–6+ · lower is better

1 Before you use it

What it is

A composite disease-activity score combining patient-reported items with an inflammatory marker — CRP or ESR. That blood component is what separates it from BASDAI.

When to reach for it

When it appears in correspondence — it tells you where the medical management currently sits, which changes what your loading and exercise programme is competing with.

2 Giving it

Scoring

Weighted combination of back pain, duration of morning stiffness, patient global, peripheral pain/swelling and CRP (or ESR). Requires a calculator and a recent blood result.

What it doesn’t measure

Function or mobility, and it needs a blood test you cannot order.

3 Reading the score

Interpreting it

Conventional cut-offs describe inactive disease, low, high and very high activity.

A physiotherapist will read this, not calculate it. It appears in rheumatology letters and it is worth being able to interpret.

Is the change real?

MCID 1.1 points for a clinically important improvement, 2.0 for a major one. Compare the change against this patient's own baseline as well as the figure. SourceMeasured in axial spondyloarthritis.

Student trap. Trying to calculate it without a CRP. Without the inflammatory marker it isn't the ASDAS — use BASDAI instead.

This is a learning tool. It does not diagnose patients and does not replace your clinical educator.
