CNFDS · Copenhagen Neck Functional Disability Scale

15 items · 5 min · 0–30 · lower is better

1 Before you use it

What it is

Fifteen yes / occasionally / no items on how the neck affects daily function. A cleaner response format than the NDI and quicker for some patients to complete.

When to reach for it

As an NDI alternative, particularly where a patient struggles with the NDI's six-option format.

Pick one neck index per episode of care and stay with it.

2 Giving it

Scoring

Each item scored 0, 1 or 2, summed to 0–30. Higher means more disability.

What it doesn’t measure

Fear-avoidance, distress or work risk. Same limits as the NDI.

3 Reading the score

Interpreting it

Interpret against baseline.

Is the change real?

No verified MCID or MDC is carried in our library for this measure, so the number on its own cannot tell you a change is real. Compare it to this patient's own baseline and say what changed functionally.

Student trap. Switching neck indices mid-episode. The scales don't convert, and the series stops meaning anything.

Official source: null. We don't host the form.

This is a learning tool. It does not diagnose patients and does not replace your clinical educator.
