Who: Middle-aged and older adults. It looks like ageing, and that is exactly the trap.
What: Hands that have stopped being reliable. A gait that has changed. Neck pain that came with them.
How it behaves: Slowly. Nothing about the onset says "refer".

The one thing to take from this page:

It is missed for years, and the mechanism of the miss is a physiotherapy-shaped problem.

FigurePopulation
Mean time from symptom onset to diagnosis2.2 ± 2.3 years — 42 surgical cases, retrospective, single centre
Modal delay1–2 years — 778 self-reported patients
Delay predicted greater disability on patient-derived mJOAp = 0.001, same cohort
Greater delay in Black or African American patientsp = 0.010, same cohort
Diagnosis was made by a family physician in4.8% of cases — neurosurgeons 38.1%, neurologists 28.6%, orthopaedic surgeons 19%
First presented to a family practitioner or an orthopaedic surgeon90.4%

And in a UK musculoskeletal triage service staffed by specialist physiotherapists, 27 cases spent an average of three months inside the service. It was suspected faster when patients had both upper-limb symptoms AND gait disturbance or pathological reflexes. It was slower when there was no gait disturbance, or a normal or incomplete neurological assessment — and slowest of all when electrophysiology was ordered to chase a peripheral neuropathy [3].

⚠️ No Australian guideline exists for this presentation. Anchored to the AO Spine clinical practice guideline [4], verified as still current. An Australian — John W. Middleton, University of Sydney — is a named author on it.

Pattern > Label. And this one barely has a label yet: diagnostic criteria and screening criteria are both still in development.

Neither ACSQHC nor RACGP has a standard or guideline for degenerative cervical myelopathy. But John W. Middleton (University of Sydney) is a named author on the 2017 AO Spine guideline this page anchors to, and David B. Anderson (University of Sydney) is senior author on the RECODE-DCM review defining where a physiotherapist sits in this pathway.
