Progressive Neuropathy / Compression Syndrome body chart
Illustrative, not diagnostic. Canva body chart for Progressive Neuropathy / Compression Syndrome.

Progressive neurological deficit is a change in function requiring assessment, not a single diagnosis. It may involve one peripheral nerve, a root, plexus, spinal cord, brain or systemic neuromuscular disease.

Document onset, speed of change, distribution and objective loss. A focal entrapment can cause weakness or wasting; a single nerve territory does not guarantee conservative care is appropriate. Bilateral or night symptoms alone do not establish central disease.

Sudden focal weakness, respiratory difficulty or new sphincter/saddle symptoms with spinal concern needs emergency assessment. Rapidly progressive symmetrical weakness needs immediate neurological assessment. Other progression requires referral urgency matched to the history and findings.

See section 6 for the action table and NICE NG127. This international guidance must be applied through local Australian pathways.

Guideline anchor

No Australian guideline covers when a physiotherapist should escalate a progressive neurological deficit.

More

Anchored to the international electrodiagnostic testing guideline review for entrapment neuropathies (Pugdahl et al. 2020, PMID 33151661) for the investigation pathway, and to the polyneuropathy guideline-informed review (Sommer et al. 2018, PMID 29478436) for the systemic differential. Both non-Australian.

PubMed Source

Both anchors are written for neurologists and clinical neurophysiologists deciding on investigation, not for a physiotherapist deciding whether to keep treating. The escalation logic on this page is drawn from the primary evidence and from the profession's own referral literature, and is labelled as such.
