1 · SNAPSHOT

Who: Kicking and sprinting athletes. Also dancers and anyone doing high-volume loaded hip flexion.
What: Iliopsoas-related groin pain, one of the four Doha entities — with or without internal snapping.
How it behaves: Anterior groin ache after kicking or sprinting. Worse with hip flexion and sit-ups. Clicking or snapping possible. Eased by relative rest. Often coexists with adductor and abdominal patterns.
The one thing to take from this page:
"Tight hip flexor" is usually a weak one. Restore load tolerance, not just flexibility — and if it snaps painlessly, treat nothing.
Two problems wearing one name
| What it is | What you do | |
|---|---|---|
| Iliopsoas-related pain | Tender deep to the inguinal ligament + painful resisted hip flexion | Treat it — load tolerance |
| Painful internal snapping | A snap that hurts | Treat the pain and the control, not the noise |
| Painless internal snapping | A snap that doesn't hurt | Treat nothing. It's noise |
⚠️ Read the surgical literature carefully. Almost all of it studies iliopsoas impingement after hip replacement, or tenotomy during hip arthroscopy. That is not the athlete in front of you.
2 · SE — THE SUBJECTIVE PATTERN (the 70%)
Five stages, in your course's order. Ask about the snap separately from the pain — they are different questions.
MAIN PROBLEM
"Deep in the front of my hip after kicking."
"It clicks when I lift my leg."
"Sit-ups are the worst."
BODY CHART
- Deep anterior, at or below the inguinal ligament.
- Mark the adductor origin, the symphysis and the inguinal canal too — the other three Doha entities.
- Mark the lesser trochanter region.
- Below the knee, or into the anterior thigh with numbness? That is a femoral nerve question.
BEHAVIOUR
| Fits iliopsoas-related | Points elsewhere | |
|---|---|---|
| Aggs | Kicking, sprinting, hip flexion, sit-ups | Cutting with adductor tenderness → adductor-related |
| Snap | With active flexion/extension | Lateral snap over the greater trochanter → external snapping hip |
| Eases | Relative rest | Nothing eases it, constant → screen |
| Severity | VAS 0–10 at best / at worst / right now | — |
| Systemic | None | Fever, unwell, night sweats → abscess |
| Weight-bearing | Able | Can't hop → bone stress |
HISTORY
- Kicking or sprinting volume, and what changed.
- Does it snap, does the snap hurt, and does it snap when nothing hurts? Three separate questions.
- Sit-up and core programme volume.
- Previous hip surgery — arthroscopy or replacement? This is the one history question that changes the whole page.
- Any anticoagulation, recent procedure, or bleeding disorder?
SPECIAL / SCREENING QUESTIONS
- Fever, feeling unwell, night sweats, recent infection?
- Any weakness in the quadriceps, or numbness over the anterior thigh? Femoral nerve.
- Can they weight-bear and hop?
- Any immunosuppression, diabetes, or intravenous drug use?

3 · HYPOTHESIS RANKING
| Competing pattern | The one discriminating feature |
|---|---|
| Adductor-related groin pain | Tender adductor origin, positive squeeze. Frequently coexists. |
| Inguinal-related groin pain | Tender pubic tubercle, conjoint tendon or canal; pain on cough and resisted sit-up. |
| Hip-related pain (FAI, labral) | Deep pinch with FADIR, reduced IR in flexion. The joint. |
| External snapping hip | The snap is lateral, over the greater trochanter — a different structure and a different page. |
| Femoral neck stress fracture | Weight-bearing pain, unable to hop, RED-S profile. |
The one that outranks everything
Non-musculoskeletal iliopsoas pathology. The deep anterior hip is also the address for:
- Iliopsoas abscess — fever, systemic illness, immunosuppression, diabetes, intravenous drug use [1][2].
- Septic iliopsoas bursitis [3].
- Iliopsoas haematoma with femoral nerve palsy — anticoagulation, a bleeding disorder, or trauma, with quadriceps weakness and anterior thigh numbness [4].
These three are why this page carries red flags.
4 · RED FLAGS — DON'T MISS THIS
This section is free, on every pattern, forever. Safety is never gated.
| Finding | What it suggests | What you do |
|---|---|---|
| Fever, unwell, night sweats with hip flexion pain | Iliopsoas abscess | Urgent medical review |
| Immunosuppression, diabetes, IV drug use, recent infection | Abscess, septic bursitis | Medical review |
| Quadriceps weakness with anterior thigh numbness | Iliopsoas haematoma with femoral nerve palsy | Urgent medical review |
| Anticoagulation or bleeding disorder with new deep hip pain | Haematoma | Urgent medical review |
| Hot, swollen, exquisitely painful hip | Septic arthritis | EMERGENCY |
| Unable to weight-bear or hop | Femoral neck stress fracture | Urgent imaging |
| Night pain or rest pain, weight loss | Malignancy, infection | Medical review |
| Progressive weakness | Neurological cause | Urgent medical review |
The rule: load-related anterior groin pain that hurts on resisted hip flexion, in a systemically well athlete who can hop, is yours. Fever, anticoagulation, quadriceps weakness or an inability to weight-bear is not.

Anchored to the Doha agreement (Weir et al. 2015, PMID 26031643), which names iliopsoas-related groin pain as one of four defined clinical entities and supplies the examination, and to the consensus on hip-related pain (Reiman et al. 2020, PMID 31959678) for where the joint sits alongside it.
The Doha agreement is terminology and definitions, not treatment. And be careful with the surgical literature here: almost all of it studies iliopsoas impingement after hip arthroplasty or tenotomy during hip arthroscopy. It is not evidence about the athlete in front of you.
